The DQ file checklist.

Hiring a CDL driver is a federally regulated act. Before first dispatch, 49 CFR Part 391 requires a complete Driver Qualification file. Missing one document is a federal violation, and DQ files are the most-cited item in DOT audits.

What the file must hold before first dispatch.

DOC 01

DOT-specific employment application

Three years of employment history, accidents, and violations. A normal job application does not satisfy this: the DOT version has required fields yours may be missing.

DOC 02

MVR from every licensing state

A motor vehicle record from each state where the driver held a license in the past three years, pulled at hire and annually after.

DOC 03

PSP report

The federal Pre-Employment Screening Program report: five years of crash history, three years of inspection history. Not technically mandatory, but expected in audits and negligent-hiring suits.

DOC 04

Previous employer verifications

Safety performance history from every DOT-regulated employer in the past three years, including drug-and-alcohol testing history. The most commonly incomplete item.

DOC 05

Medical examiner's certificate

The current med card, from an examiner on the FMCSA National Registry, with the National Registry number verified.

DOC 06

Pre-employment drug test + Clearinghouse query

A negative DOT pre-employment drug test result AND a full Drug & Alcohol Clearinghouse query before the first safety-sensitive function. Annual limited queries after.

DOC 07

Road test certificate or CDL equivalent

A documented road test, or the CDL itself where accepted as equivalent, plus a copy of the license.

DOC 08

English proficiency, 391.11(b)(2)

The driver must read and speak English well enough to converse, understand signs, and complete reports. Actively enforced since 2025: an out-of-service trigger at roadside.
